We are pleased to announce that you can now access health services from St. Luke’s Surgery on the new NHS Wales App. The App is a simple and secure way to: book routine appointments, order repeats and view parts of your medical record. To access the App you must have a fully verified NHS Login, or a valid photo ID to prove who you are and be aged 16 or over. Download the App today or use the desktop web version available.

FEAR OF FLYING

We have taken the decision to stop prescribing Diazepam outside the terms of its licence, including as a sedative for aircraft flights.  This policy decision has been made by the GP Partners and will be adhered to by all prescribers working in the practice.

In-house Physiotherapy Assessment & Advice Service

The Practice now holds a weekly physiotherapy assessment and advice service.  If you are over 16 and need advice or to be assessed for a muscle, bone or joint problem or injury, please contact us and we can arrange an appointment for you to see our Physiotherapist.  Clinics are held every Tuesday morning at the surgery.

My Health On-Line

The Practice is  registered with My Health on Line (MHOL) allowing registered patients to book or cancel an appointment, and  order repeat prescriptions on-line.  

To register for on-line services - please ask a receptionist for the appropriate forms, or download a copy using the links below.  Alternatively, someone can collect the forms and register on your behalf, with your permission.

Booking and Cancelling appointments on line -  When you are registered for on-line services you will be able to book and also cancel any unwanted appointments. Full instructions on how to do this are provided in the User Guide available to download above.

Welsh GP Record - NHS Wales is making changes to the way your information is used for your care

  • Medical Assessment Unit
